Returning Group Forms

Steps to re-register a returning group:

Step 1:
Fill out CSC France-Merrick Cover Sheet, CSC Returning Group Bio, CSC Group Budget

Step 2:
Turn in hard copies of the France-Merrick Cover sheet (signed), Returning Group Bio, and Group Budget at the CSC (3103 N. Charles Street). 

NEW PROCEDURE: Student group leaders MUST sign up for a 30 minute appointment with a CSC staff member. Appointment sign-up sheets are available at the reception desk. At the appointment, group leaders will discuss their goals for the year and be given a copy of their host site's feedback form (if applicable).

Step 3:
Email copies of the Returning Group Bio and Group Budget to volunteer@jhu.edu with your group name in the subject line.
